I'm afraid that there is no built-in feature for this. Yet, as a workaround solution I can offer you to use one of the applications — "Custom Redirect for 'Thank You' Page". It will help you to redirect your customers to a custom "Thank you" page after they complete their order. You can install it from here:
https://www.ecwid.com/apps/tools/custom-thank-you. Please, note that applications are available on all of the paid plans.
After you install it, please open this app. Then you can paste any URL in the field and your customers will be redirected to that page after they place their order in the store. Thus, you can set up a redirect to a custom form, for example, Google Form (
https://docs.google.com/forms), where your customers will complete a questionnaire.
You will be able to access the responses in a Google Spreadsheet (or download them in a CSV file). Also, you will receive email notification about new responses.
